During Ramadan, fasting from dawn to sunset presents unique challenges for those using Mounjaro & Wegovy, as it affects meal timing, blood sugar levels and dosing schedules. This FAQ guide provides guidance on how to safely use Mounjaro whilst fasting, including tips on timing injections, managing potential side effects, and recognising signs of low or high blood sugar. 

Can I take my injectable weight loss medication while fasting, and is it safe?

You can continue taking your medication while fasting, but there are important safety considerations to consider. Prolonged fasting increases the risk of low blood sugar and dehydration, which can be worsened by injectable weight loss medications such as GLP-1 receptor agonists (Mounjaro, Wegovy, Saxenda). We do not generally recommend taking these medications while fasting for these reasons without reading this guide carefully. However, if you choose to continue, it is important to follow the safety guidelines below.

Will injecting Mounjaro break my fast?

No, according to Islamic scholars, subcutaneous injections that are not nutritional and do not
provide sustenance therefore do not break the fast. Since GLP-1 medications are not a form of
nutrition, using them during fasting hours is generally considered permissible. However, if you
are unsure, we recommend consulting your Imam for guidance.

Should I consider stopping my GLP-1 during Ramadan?

If you're concerned about taking weight loss medication while fasting during Ramadan, it's okay to pause it for the month. Stopping for a short period won’t derail your long-term weight loss goals. Since you’ll be fasting, it’s unlikely that you’ll gain weight. Evidence has shown that when people with obesity fast during Ramadan, they show a reduction in body fat percentage. If you decide to stop taking your medication for the entire month of Ramadan, you may need to adjust your dosage when restarting:

  •  If you are on a dose of 10mg or higher, restart at 5mg.
  •  If you are on a dose lower than 10mg, restart at 2.5mg.

If I do decide to continue, when should I take my injection during Ramadan?

For best results, it is recommended to take your injection after suhoor rather than after iftar, as this minimises any interference with your ability to eat and stay hydrated.

How should I manage hydration while fasting and taking weight loss injections?

Hydration is crucial when using GLP-1 medications, especially when there is an increased risk of dehydration like during Ramadan. During the non-fasting hours, aim to drink at least 2-3L of water. This will help reduce the risk of dehydration and associated symptoms like dizziness, headaches, and fatigue. We also recommend the use of electrolytes.

What should I eat during non-fasting times?

It is important to fuel your body properly and choose your food wisely when taking weight loss injections, and this becomes even more important during periods of fasting. To support your body during Ramadan and while still pursuing weight loss goals, we recommend you focus on:

  • High-protein foods like eggs, chicken, fish, or legumes to maintain energy levels and
    help you feel full.
  • Fiber-rich and nutritious foods such as whole grains, fruits, and vegetables to aid
    digestion and fullness.
  • Healthy fats from sources like nuts, seeds, and olive oil to help with satiety.
  • Slow-digesting complex carbohydrates like oats, brown rice, brown pasta and whole
    wheat to prevent blood sugar spikes and provide sustained energy.
  • Avoid excessive sugars, fried, oily, and processed foods, as they can lead to fluctuations
    in blood sugar and slow down your ability to digest food.
  • Don’t overeat, as eating too much at either meal can lead to discomfort and fatigue.
  • Focus on moderate portion sizes which will allow you to eat regularly throughout non-
    fasting periods.

It is also important to make smart food choices tailored to Iftar and Suhoor. For Iftar, focus on rehydrating, and include protein-rich foods like chicken, whole grains like brown rice or quinoa, and fresh vegetables for fiber. For Suhoor, choose complex carbs like oats or whole grain bread to provide sustained energy, protein from eggs or Greek yogurt to keep you full, and healthy fats from avocados or nuts for long-lasting satiety. How should I approach exercise while fasting on weight loss injections?

Some people might find it tempting to reduce the amount of exercise they are doing during Ramadan. For most people, starting a new exercise programme is unrealistic during Ramadan, therefore we recommend maintaining your current levels of activity depending on your energy levels.

If you are exercising while using weight loss medication and fasting, we recommend the
following:

  • Timing: Schedule your workouts after Iftar, when you've replenished your energy and
    hydration levels. This will help you perform better and avoid dehydration or fatigue.
  • Intensity: Opt for moderate-intensity exercises, such as walking, stretching, or light
    aerobic exercises, that match your energy levels. Overexerting yourself could lead to
    exhaustion or injury.
  • Listen to your body: Pay attention to how your body feels during exercise. If you
    experience dizziness, fatigue, or discomfort, it's important to stop and rest.
  • Stay hydrated: Drink plenty of water during non-fasting hours to stay hydrated. This is
    essential for exercise performance, recovery, and safety on weight loss medication.

What symptoms of low blood sugar and dehydration should I watch for while fasting on GLP-1 medication, and how should I respond?
